Tikanga and Kawa: The governing tikanga and kawa of all Whakapai Hauora services is Rangitāne.

Primary Function: To contribute to Whakapai Hauora Vision and Mission whilst contributing and providing Whānau Ora Practice Nurse services including care coordination.

Summary of Position: To ensure that efficient, effective, and culturally appropriate Whānau Ora Practice Nurse services are provided to Rangatahi in residence at Te Au Rere.

A whānau-centred approach is a culturally grounded, holistic approach focused on improving the wellbeing of whānau and addressing individual needs within a whānau context. Key Tasks Performance Standards

Clinical Nursing Services

To provide primary health care nursing services as follows:

• Delivery of Nurse clinics: Nurse triage and assessment of all Young Persons booked into the Nurse clinic; attending to urgent GP tasks, script requests and fax requests. Referrals as appropriate etc.

• Health screening and health promotion: reviews as required, health risk assessments, screening as required, immunisations, brief smoking advice etc.

• Ensuring that all Young Person tasks allotted by the GP is completed.

• Follow-up of Young Person test results and communication with Young Persons where appropriate.

• Undertaking treatment options and carrying out appropriate procedures and clinical Nursing interventions.

• Setting up and assisting with minor surgery and other procedures as required.

• Complying with all Whakapai Hauora policies and procedures.

Maintaining and Improving Health Outcomes To provide timely clinical and culturally appropriate health services through:

• Ongoing health education and advice to improve health and prevent disease.

• The use of recall and reminder systems on Medtech as appropriate, referral to national screening programmes.

• Education to assist Young Persons to reduce or change risky and harmful lifestyle behaviours.

• The provision of contraceptive advice and sexual health services.

• Delivery of immunisations.

• Collaborating with THINK Hauora and Public Health Providers in the prevention and control of communicable diseases for individuals and families and reporting to relevant Public Health Providers.

• Health promotion to Young Persons linking to Public Health Programmes at a local, regional and national level, and where appropriate, using such programmes to target specific populations.

• To participate in all meetings related to general practice requirements.

Whanau Ora Practice Nurse Administration Whānau Ora Practice Nurse administration is not limited to, but also includes the following:

• All Young Person note entries (e.g. Nurse consults, Nurse phone calls, Nursing cares, Young Person reviews, care plans and action plans are recorded on Medtech in a timely manner and are up to date.

• All Young Person recalls are up to date.

• All new Young Person records received from previous GPs are entered accordingly into Medtech.

• Young Person transfers in and out.

• Register all new Young Persons on MedTech including NHI numbers.

• Upload Young Person consent forms onto MedTech once signed.

• Upload Young Person’s full health summary onto MedTech and send to appropriate Oranga Tamariki Case Leaders.

• Upload consultation notes to daily the Young Person’s record on MedTech.

• Uploading Forensic consultation notes into Young Person’s files.

• Complete Young Person’s note entries on MedTech Evolution in a timely manner.

• Become ACC accredited then through Medtech forward all relevant ACC documentation through to head office in Hamilton.

Provision of Mental Health Nursing Services to support Rangatahi towards achieving Whānau Ora.

This shall include:

• The provision of clinical support as required;

• Delivery of programmes to enhance Mental Health Wellbeing and support those with addictions, such as the Smashed and Stoned programme;

• Completion of Health Care Plans for Rangatahi that meet the needs of Te Au Rere A Te Tonga;

• Managing any barriers to informed decision making that are affecting the Rangatahi ability to achieve better health status;

• Work towards decreasing the stigma towards people with mental health or addictions which may lead to discrimination;

• Liaise with other staff in Te Au Rere A Te Tonga as necessary;

• Provision of brief interventions where applicable;

• Maintenance of networks with outside organisations as required; and

• Operate in a multi-disciplinary way.

ACC Financial Claims

• ACC claiming and all subsidies from any funding agency must be adhered to.

Infection Control • Comply with all infection control policies and procedures.

• Daily maintenance of the autoclave and log.

• Clinic infection control audits undertaken 6 monthly.

• Assist with disinfection and sterilisation of reusable surgical equipment.

• Disposing of clinical waste in accordance with all health and safety regulations.

• Clean down of examination beds and bench surfaces after use.

• Assisting with clean up after minor procedures.

• Comply with all infection control policies and procedures.

• Compliance with BCWH Pandemic Plan.

Cold Chain Management • Complying with all requirements as per Whakapai Hauora Cold Chain policy.

• Daily vaccine fridge temperature monitoring.

• Vaccine stock maintenance.

• Undertake 2 yearly cold chain accreditation.

Drug Restock and Supplies Undertake the routine restocking of:

• Crash Trolley and Oxygen cylinder;

• MPSO and medicine cupboard stock;

• Doctor’s Bag;

• Controlled drugs; and

• Weekly order of liquid nitrogen.

Undertake stock audits/checks of the following:

• Crash Trolley and Oxygen cylinder (weekly);

• Controlled drugs (3 monthly however, restock whenever required);

• Medicine cupboard stock (6 monthly, however, restock whenever required); and

• Doctor’s bag (6 monthly, however, restock whenever required).

Clinical Supplies • Ongoing check of supplies for adequate maintenance of all stock.

• Monthly ordering and restocking as required.

• Undertake 6 monthly stock audit.

• Undertake annual checks of First Aid Kits across the Rōpu and for the vehicles.

Laboratory Supplies • Checking, ordering and restocking of laboratory stocks on a regular basis.

OSH • Arrange for annual calibration of all clinical equipment as required.

• Arrange for annual testing of all clinic RCDs as required.

• Participate in 6 monthly Fire Drills.

• Comply with all Te Au Rere OSH policies and procedures.

CQI and Accreditation • Participate in EQuIP6 Accreditation Programme as required.

• Participate in audits as required.

• Compliance with General Practice Service Plan, Quality Plan, Strategic Plan, Health & Safety Plan, Risk Management Plan.

Professional Development • Ensure that all Whānau Ora Practice Nurse training updates are maintained (e.g. immunisation, CPR, etc.)

• Annual Practicing Certificate is maintained and up-to-date.

1. Is a Registered Nurse, competent and flexible enough to participate as a team member in a Māori driven and managed setting.

3. Has a commitment to providing culturally safe, professional, innovative, compassionate, skilled nursing that meets whānau, individual and Māori needs.

4. Demonstrates a style of care, which facilitates active participation and empowers clients to make choices regarding their health care.

5. Has an understanding of tikanga Māori and the political, social and legal environments they are working within.

6. Practices within a philosophy and environment, which recognises Te Tiriti o Waitangi, and in a manner, which is culturally safe.

7. Has demonstrated confidence in own judgement and a willingness to express an independent viewpoint.

8. Has demonstrated the ability to persevere though periods of heavy workloads in stressful situations.

9. Has the ability to analyse situations and utilise cause and effect, systematic approaches to problems.

10. Has an open approach to the structures and protocols of other cultures and a willingness to use these.

11. Has the ability to organise workload by efficient use of time, setting targets and achieving them.
