Summary:

The Community Support Worker will play a vital role in providing assistance and support to individuals within the community. This position requires a compassionate and empathetic individual who can effectively communicate and build relationships with diverse populations. The Community Support Worker will collaborate with various stakeholders to ensure the well-being and integration of individuals into the community.

Responsibilities:

  • Provide direct support to individuals in the community, including emotional support, advocacy, and crisis intervention.
  • Assist individuals in accessing community resources and services, such as healthcare, housing, employment, and education.
  • Develop and implement personalized support plans based on individual needs and goals.
  • Collaborate with families, caregivers, and other professionals to ensure a holistic approach to support.
  • Facilitate group activities and workshops to promote socialization and skill development.
  • Maintain accurate and confidential documentation of client interactions and progress.
  • Stay updated on relevant community resources, policies, and regulations to provide accurate information and guidance.
  • Participate in team meetings, training sessions, and professional development activities.

Qualifications:

  • Bachelor’s degree in social work, psychology, or a related field (or equivalent experience).
  • Proven experience working with diverse populations, including individuals with disabilities, mental health challenges, or other support needs.
  • Strong interpersonal and communication skills, with the ability to build rapport and establish trust.
  • Knowledge of community resources and services, and the ability to navigate and advocate within the system.
  • Ability to work independently and as part of a team, with excellent problem-solving and decision-making skills.
  • Proficient in using technology and software for documentation and communication purposes.
  • Valid driver’s license and access to reliable transportation.
