Yadu Health Aboriginal Corporation is a not-for-profit, community-controlled health service providing primary health care to Aboriginal communities in Ceduna, Koonibba and surrounding areas.

For more than 30 years, Yadu Health has delivered affordable, accessible and culturally safe health care, with a strong focus on improving the health and wellbeing of Aboriginal and Torres Strait Islander people.

Our services are delivered through a multidisciplinary team of health professionals, including Aboriginal Health Workers and Practitioners, nurses, GPs, visiting specialists and allied health professionals. Yadu Health also operates a fully accredited 24-hour residential aged care facility.

About the Role

Yadu Health is seeking dedicated and compassionate Aboriginal Health Workers / Practitioners to join our Primary Health Care team.

We have multiple permanent full-time positions available for people passionate about improving Aboriginal health outcomes and providing timely, culturally appropriate and holistic care within both community and clinical environments.

Aboriginal Health Workers and Practitioners play a vital role in connecting clients, families, communities and health services. You will work closely with Yadu’s multidisciplinary team to provide culturally safe health care, education, support and advocacy while helping clients navigate and access the services they need.

Key Responsibilities

Key responsibilities include:

• Support Aboriginal and Torres Strait Islander clients to access comprehensive, culturally safe primary health care and health education.
• Undertake health assessments, screenings and health promotion activities, including supporting early detection and management of health conditions.
• Develop and coordinate individual care for clients, including complex cases, using recognised care plans and evidence-based clinical guidelines.
• Initiate, coordinate and support the review of health checks, care plans, referrals and patient recalls.
• Prepare and support clients before and after procedures.
• Build strong, respectful relationships with Aboriginal and Torres Strait Islander clients, families and communities.
• Act as a cultural liaison between clients, families and health professionals, helping ensure services are culturally appropriate and responsive.
• Support clients to navigate health care systems and connect with appropriate health and community services.
• Participate in community meetings, outreach, events and health promotion activities.

About You

• Identification as an Aboriginal and/or Torres Strait Islander person (desirable).
• A Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice or Community), or equivalent qualification.
• Experience in a similar health or community-based role, with an understanding of regional health services.
• An in-depth understanding of Aboriginal and Torres Strait Islander cultures, communities and culturally safe health care.
• Strong knowledge of the health issues affecting Aboriginal and Torres Strait Islander people, particularly those living in rural and remote communities.
• Strong interpersonal and communication skills, with the ability to build trusted relationships with clients, community members, colleagues and external stakeholders.
• The ability to work effectively both independently and as part of a multidisciplinary health care team.
• Strong computer literacy and the ability to maintain accurate client records and prepare professional documentation.
• A current Class C Driver Licence or equivalent.
• A valid Working with Children Check, National Police Check and Aged Care Sector Employment Check.
• A current First Aid Certificate, or willingness to obtain one.
• Current immunisations, or willingness to participate in Yadu Health’s staff immunisation program.

Previous experience working within an Aboriginal Community Controlled Health Organisation, Aboriginal health service, health care or community services environment will be highly regarded.
