About the Role
As a Care Partner, you’ll build trusted relationships with Elders and work alongside them to understand their needs, goals and preferences.
You’ll manage a client caseload, develop and review practical care plans, and coordinate supports that help Elders remain independent and connected at home. You’ll monitor how things are going, respond to changes early and ensure services remain appropriate to each person’s goals and available funding.
Working with families, authorised supporters, the Wyanga team and external providers, you’ll help Elders navigate their care and maintain continuity of support.
What you’ll do
- Undertake assessments and home visits to understand each Elder’s needs, circumstances and goals.
- Develop, implement and review care plans with Elders and their chosen supporters.
- Coordinate services and referrals, following up to ensure supports are meeting the Elder’s needs.
- Monitor changes in wellbeing, identify risks and escalate concerns appropriately.
- Help Elders understand their services, funding and available options.
- Monitor service use and available budgets, working with the team to address gaps or changing needs.
- Maintain accurate, timely client records, case notes and care documentation.
- Work collaboratively to deliver culturally safe, respectful and person-centred care.
About You
You’ll bring experience in aged care, community services or a related field, with the confidence to manage a caseload and follow things through.
You are someone who:
- Builds strong, respectful relationships with Elders, families and community.
- Understands culturally safe practice and respects each person’s choices.
- Uses sound judgement and recognises when to seek support or escalate a concern.
- Manages competing priorities while keeping care plans, records and follow-up actions current.
- Communicates clearly and works well with colleagues and external providers.
- Is comfortable using digital systems to document and coordinate care.
Experience in care planning, case management or service coordination will be highly regarded.
Qualifications & Requirements
- Certificate IV or higher in Aged Care, Community Services or a related field, or equivalent relevant experience.
- Current National Police Check.
- Current First Aid and CPR certification, or willingness to obtain.
- Current driver’s licence.
- Working knowledge of community aged care, particularly Support at Home and the Commonwealth Home Support Programme, is desirable.
