Support Coordinator – NDIS

Perth WA | Part-Time (0.5 FTE) | 24 Months Contract | Hybrid work available$43.00 – $45.00 per hour + super

Space for Life by BGSR is seeking an experienced Support Coordinator to support participants in implementing their NDIS plans and building capacity for greater independence, inclusion and community participation.

This role works directly with participants funded under the National Disability Insurance Scheme to coordinate supports, strengthen connections to community and mainstream services, and promote person-centred outcomes aligned with participant goals.

The position requires a proactive and organised professional who can manage a caseload, build strong stakeholder relationships, and ensure participants are supported to maximise the benefits of their NDIS plans.

Key Responsibilities

• Coordinate and implement participant NDIS plans, ensuring supports align with participant goals and funding

• Connect participants with appropriate community, mainstream and specialist support services

• Monitor service agreements, budgets and funding utilisation to ensure effective plan implementation

• Prepare reports and support participants through NDIS plan reviews and meetings

• Maintain accurate case notes and documentation in accordance with NDIS requirements and organisational policies

• Build strong working relationships with participants, families, providers and community partners

• Identify and respond proactively to risks, issues or barriers impacting participant outcomes

Selection Criteria

Essential

• Relevant qualification in Community Services, Disability, Social Work or similar

• Minimum 2 years’ experience in disability services, case management or support coordination

• Demonstrated understanding of the NDIS and participant plan implementation

• Experience working with people with disability and/or chronic or persistent mental illness

• Strong knowledge of person-centred and recovery-oriented practices

• Excellent written communication, reporting and organisational skills

• Strong computer skills including Microsoft Office and Client Management Systems

• Ability to manage workload independently in a complex and fast-paced environment

Desirable

• Experience in Support Coordination

• Knowledge of community and mainstream support services

• Experience supporting participants with psychosocial disability

Requirements

• Current NDIS Worker Screening Check (or willingness to obtain)

• Completion of NDIS Worker Orientation Module

• Current Driver’s Licence and willingness to use personal vehicle for work purposes

• Evidence of secure home workspace

• Australian working rights
