Position Purpose:
Reporting to the Operations Manager, the purpose of your role is to support older people receiving in-home aged care services through South West Community Care to maintain their independence, wellbeing, safety and quality of life. The role combines person-centred occupational therapy assessment and intervention with the coordination of allied health services. With a strong focus on prevention, early intervention and reablement, the position improves access to timely, integrated allied health support under the Support at Home program.
Working collaboratively with clients, Care Partners, clinical professionals, service delivery teams and external allied health providers, the Occupational Therapist coordinates responsive, evidence-informed services tailored to each client’s needs, goals and preferences. The role supports clients to optimise their functional capacity, manage risks and remain safely and independently in their own homes for as long as possible.
Position Responsibilities:
As an Occupational Therapist you will be responsible for and assisting with:
Client Assessment & Intervention
- Conduct comprehensive, person-centred occupational therapy assessments in clients’ homes, considering functional capacity, activities of daily living, environmental and safety risks, equipment needs, and opportunities to enhance independence.
- Develop practical, goal-directed intervention plans with clients and their Care Partners, reflecting assessed needs, informed choice, personal preferences, and agreed wellness, reablement and independence outcomes.
- Assess, trial and recommend assistive technology, home modifications and adaptive strategies that are safe, clinically appropriate and aligned with client goals and applicable funding arrangements.
- Educate clients, families and relevant personnel in safe and effective strategies that support daily living, independence and the appropriate use of recommended equipment.
- Monitor and evaluate the effectiveness of interventions, reassessing clients when their needs, risks, goals or circumstances change.
- Identify needs outside the occupational therapy scope of practice and facilitate timely referral to appropriate allied health, medical or other clinical professionals.
Care Coordination & Program Delivery
- Collaborate with Care Partners, Care Coordinators and other professionals to coordinate safe, integrated and person-centred care.
- Provide occupational therapy advice across CHSP and Support at Home services, promoting culturally safe practice that upholds dignity, informed choice and supported decision-making.
- Manage a client caseload to support timely assessment, risk-based prioritisation, accurate documentation, program compliance and achievement of agreed client outcomes.
- Ensure assessments, interventions and clinical records comply with professional standards, organisational policies and applicable aged care requirements.
Complex Clinical Advice & Delegated Decision-Making
- Provide occupational therapy advice and support proportionate decision-making where client rights, preferences and choices must be considered alongside identified risks and duty-of-care obligations, escalating matters in accordance with organisational policy and delegation.
Workforce Capability & Professional Support
- Build workforce capability through clinical guidance, coaching, mentoring and education for Care Partners, Care Coordinators, clinical personnel and other relevant employees.
- Promote consistent, evidence-informed practice and strengthen workforce understanding of occupational therapy, wellness, reablement and restorative approaches.
Clinical Governance & Continuous Improvement
- Lead occupational therapy quality-improvement activities, using client outcomes, audit findings, incidents, feedback and contemporary evidence to strengthen clinical practice and service delivery.
- Contribute to organisational policy and practice development and support preparedness for audit, accreditation and regulatory review.
- Engage with internal and external stakeholders and provide escalation within professional scope and organisational delegations.
To thrive in this role you will have:
- Tertiary qualification in Occupational Therapy, with current registration as an Occupational Therapist with the Australian Health Practitioner Regulation Agency (AHPRA), and demonstrated experience providing occupational therapy assessments and interventions, preferably within community health, rehabilitation or aged care.
- Demonstrated understanding of contemporary aged care practice and the delivery of person-centred care that promotes client choice, dignity, independence, wellbeing and quality of life, while balancing individual preferences with identified clinical needs and risks.
- Sound understanding of ageing, functional decline, falls prevention, reablement and maintaining independence.
- Demonstrated ability to assess client needs and develop practical, person-centred interventions and remain responsive to clients’ assessed needs, goals and changing circumstances.
- Demonstrated experience responding to changes in client condition and emerging clinical risks, clinical governance, risk management and professional documentation requirements.
