WHO ARE WE?
The Rev. Bill Crews Foundation (BCF) is one of Sydney’s most respected frontline charities, supporting individuals and families experiencing hardship, homelessness and social disadvantage for almost 40 years. Through a range of integrated services including healthcare, social support, education, food relief and community programs, we work to create pathways toward stability, wellbeing and independence.
Our Social Health and Wellbeing (SHW) team provides compassionate, practical support to people facing complex challenges. Whether someone requires immediate assistance, advocacy, referral services or short-term case management, our team is committed to helping people access the support they need with dignity and respect.
THE ROLE
We are seeking a compassionate and dedicated Intake Case Worker to join our Social Health and Wellbeing team. As one of the first points of contact for clients accessing our services, you will play a critical role in assessing needs, responding to immediate concerns and connecting individuals with appropriate internal and external supports. Working within a multidisciplinary and client-focused environment, you will provide short-term case management and help people navigate a range of social, health and community services.
WHAT YOU’LL DO
Reporting to the Client Services Coordinator, you will:
- Be the first point of contact for new and returning clients, conducting intake and needs assessments.
- Provide short-term, strengths-based case management focused on practical outcomes.
- Connect clients with internal services including meals, healthcare, dental services, social support programs and community initiatives.
- Facilitate referrals to external services including housing, mental health, family support and alcohol and other drug (AOD) programs.
- Advocate on behalf of clients where appropriate to assist with accessing essential services and support.
- Maintain accurate, confidential and timely client records.
- Build and maintain relationships with community organisations and referral partners.
- Participate in team meetings, supervision and ongoing professional development activities.
- Work collaboratively with staff and volunteers across BCF’s frontline services.

You are a motivated and empathetic practitioner who is passionate about supporting vulnerable people and achieving positive client outcomes. You are adaptable, organised and capable of working effectively in a fast-paced environment.
You will also bring:

- Minimum two years’ experience working with vulnerable individuals and families within community services, housing, homelessness, health or related sectors.
- Demonstrated understanding of case management principles and client-centred practice.
- Knowledge of housing pathways, community resources and referral networks.
- Experience supporting clients experiencing mental health concerns and/or alcohol and other drug (AOD) issues.
- Excellent communication, interpersonal and relationship-building skills.
- Strong administration and case note documentation capabilities.
