Responsibilities:
- Provide direct support to individuals in the community, including emotional support, advocacy, and crisis intervention.
- Assist individuals in accessing community resources and services, such as healthcare, housing, employment, and education.
- Develop and implement personalized support plans based on individual needs and goals.
- Collaborate with families, caregivers, and other professionals to ensure a holistic approach to support.

Qualifications:

- Strong interpersonal and communication skills, with the ability to build rapport and establish trust.
- Knowledge of community resources and services, and the ability to navigate and advocate within the system.
- Ability to work independently and as part of a team, with excellent problem-solving and decision-making skills.
- Proficient in using technology and software for documentation and communication purposes.
- Valid driver’s license, reliable transportation can be provided.
