Description
At Life Care, we are driven by our vision to support older South Australians in shaping their own positive ageing, expressed through meaningful connections and participation. For over 75 years, we’ve embraced ageing, driven by values of respect, integrity, engagement, teamwork, and excellence. Join us to make a positive impact in the lives of older South Australians and grow your career within a team that truly values people.
The Role:
As we continue to grow, we are looking for a dedicated and experienced Physiotherapist to join our multi-disciplinary team. This role offers the opportunity to work predominantly in the community across Adelaide’s southern suburbs, with some work based at our Payneham clinic, providing high-quality care to aged care clients, including those supported through Support at Home (SAH), Commonwealth Home Support Program (CHSP), NDIS and private services.
You’ll work with older adults in their own homes, helping them improve mobility, rebuild confidence and maintain independence. Your caseload may include falls prevention, rehabilitation after illness or surgery, and management of musculoskeletal, neurological and chronic conditions.
This role suits someone comfortable making clinical decisions independently, who enjoys adapting their approach to different people and home environments. You’ll have the support of our allied health colleagues and administrative team, with opportunities to discuss cases and share ideas.
Services are delivered across Support at Home, CHSP, NDIS and private funding arrangements, with some clinic-based work where required.
Key Responsibilities:
- Understand what matters to each client: assess their mobility, function and everyday challenges, and agree on meaningful, achievable goals.
- Deliver practical care at home: provide individualised treatment and exercise programs that make the most of the client’s abilities, preferences and home environment.
- Support safe mobility and independence: assess falls risk, transfers and walking, and recommend or trial appropriate mobility aids within your scope.
- Review and adapt care: monitor progress, adjust treatment and recognise when further advice, referral or a different approach would benefit the client.
- Build confidence and self-management: help clients, families and carers understand practical ways to support movement, exercise and daily activities.
- Stay connected with the wider team: communicate relevant findings and progress with colleagues, GPs and care coordinators to support continuity of care.
- Manage your allocated caseload: maintain timely clinical records and contribute ideas that improve the client experience.
