Future Care Partner Opportunities

As a Care Partner, you would work across clients’ homes and community settings throughout Southeast Sydney.

This is a field-based position – not a traditional desk-based role. You would support clients funded through the NDIS, including people living with disability and individuals recovering from injury through iCare NSW.

Your focus would be ensuring every client receives personalised, respectful and well-coordinated support aligned with their goals, preferences and funding requirements.

What You Could Be Doing

As a Care Partner, your responsibilities may include:

• Conducting holistic assessments covering clients’ physical, psychosocial, cultural and environmental needs
• Developing, implementing and reviewing individualised care plans centred on each client’s goals, safety, wellbeing and quality of life
• Acting as the primary point of contact for clients, families and carers
• Advocating for clients’ rights, dignity, choices and preferences
• Conducting regular check-ins, progress reviews and site audits to maintain quality, safety and satisfaction
• Reviewing client notes, incidents and service trends to identify risks and escalate concerns appropriately
• Completing accurate and timely reporting for the NDIA and iCare NSW in line with relevant program standards
• Building strong relationships with clients, representatives and industry partners
• Supporting the growth of your client portfolio by confidently sharing the Leora story

About You

We would be interested in hearing from people who bring:

• A relevant qualification in allied health, disability, aged care or another community-services field
• Experience supporting people through the NDIS, disability or community-care sector
• Community engagement, networking or stakeholder engagement experience
• Strong relationship-building and stakeholder-management skills
• Excellent verbal, written and presentation skills
• Sound organisation, judgment and decision-making abilities
• Confidence working independently in a fast-paced, community-based environment
• A valid Australian Driver Licence, as regular community travel is essential
• A genuine commitment to improving the lives of people living with disability

You will also need a valid NDIS Worker Screening Check or be willing to obtain one.
