About the Opportunity

As we grow, our Care Partner roles offer a refreshing shift in pace and purpose. These are dynamic, field-based, community roles not desk jobs. You’ll be out supporting older Australians in their own homes and local settings through thoughtful assessment, person-centred planning and hands-on coordination — helping people stay independent, safe and connected to what matters to them.

Portfolios focus on clients funded under Supports at Home (Aged Care), ensuring support is personalised, respectful and aligned with funding requirements.

What the Role Looks Like

As a Care Partner, you’ll:

• Conduct holistic assessments covering physical, psychosocial, cultural and environmental needs
• Develop, implement and review individualised care plans built around each client’s goals, wellness, independence, safety and quality of life
• Ensure reassessments happen at regular intervals and key transition points — hospital discharge, change in condition, change in housing
• Be the primary point of contact for clients, families and carers, advocating for their rights, dignity and preferences
• Oversee services delivered by third-party providers for clients funded under Supports at Home
• Report accurately and on time to My Aged Care and other relevant agencies in line with program standards
• Grow your portfolio by sharing the Leora story with clients, representatives and industry partners

About You

You’ll bring:

• A qualification in allied health, aged care, disability or a related community field
• Community engagement experience, including networking and stakeholder engagement
• Strong stakeholder management and relationship-building skills
• Excellent verbal, written and presentation communication
• Strong organisation and decision-making skills, especially in fast-paced environments
• A valid Australian Driver Licence (community travel is essential)
• A genuine commitment to helping older Australians live well at home

You’ll also need a valid NDIS Worker Screening Check (or willingness to obtain one).
