At Impact, we know everyone’s journey to employment looks different. We’re looking for a Case Management Consultant who can meet people where they’re at, understand what’s getting in the way and help them find a way forward.
Whether you have experience in employment services, community services or another people-focused role, or you’re looking to bring your skills into a new industry, we’d love to hear from you.
What you’ll do
You’ll manage a caseload of participants who may be experiencing vocational and non-vocational barriers, providing individualised support to build engagement, confidence and work readiness.
You’ll:
- Build genuine relationships and understand each participant’s individual circumstances, strengths and barriers
- Develop practical strategies to increase engagement and progress towards employment
- Connect participants with Allied Health, community services and other supports when needed
- Support participants to build confidence, capability and greater independence
- Provide post-placement support to help participants succeed and stay in employment
- Maintain accurate and timely case notes, documentation and compliance requirements, and
- Work closely with your team and external providers to achieve the best outcomes for participants.
