About the Role
We are seeking an experienced and passionate NDIS Area Coordinator to provide coordination and oversight of services for participants with complex needs, psychosocial disability, mental health conditions, high-intensity support requirements, and challenging support environments.
In this role, you will play a key part in ensuring the delivery of safe, high-quality, person-centred supports by coordinating participant services, overseeing support workers, and working collaboratively with participants, families, guardians, support coordinators, allied health professionals, and other key stakeholders. You will be responsible for supporting participants to achieve their NDIS goals, maintain their independence, and enhance their quality of life through responsive and outcome-focused service delivery.
Based in Perth, Western Australia, this role involves regular participant home visits, stakeholder meetings, service reviews, and on-site support to frontline teams to monitor participant wellbeing, service quality, staff performance, and overall support success.
Key Responsibilities
- Coordinate participant supports in line with individual needs, NDIS plans, and goals.
- Conduct participant assessments, reviews, and home/community visits.
- Coordinate complex and high-intensity supports.
- Build strong relationships with participants, families, guardians, support coordinators, and external providers.
- Monitor participant outcomes, incidents, risks, and service quality.
- Maintain accurate participant records, reports, and NDIS documentation.
- Coordinate rostering, scheduling, and workforce requirements.
- Work collaboratively with multidisciplinary teams and service providers.
- Identify and actively engage with potential participants, referral sources, and community networks to support the growth of participant services.
- Build and maintain professional referral relationships with support coordinators, allied health professionals, community organisations, and other relevant stakeholders.
- Identify opportunities to connect suitable participants with our services while ensuring a strong focus on participant needs, suitability, and quality of care.
- Ensure services are delivered in line with NDIS Practice Standards and compliance requirements.
Qualifications and Experience
- Minimum 2 years’ proven experience in NDIS coordination, support coordination, disability services, case management, or a similar role.
- Relevant tertiary qualifications in Nursing, Social Work, Social Welfare, Disability, Community Services, or a related field will be highly regarded.
- Registered Nurse (RN) qualification is highly preferred, particularly with mental health or complex care experience.
- Experience in NDIS coordination, disability services, case management, or a similar role.
- Strong understanding of NDIS processes, documentation, and compliance.
- Experience supporting people with complex or psychosocial needs.
- Knowledge of mental health, positive behaviour support, and high-intensity supports.
- Excellent communication, organisation, and stakeholder management skills.
- Strong problem-solving and decision-making abilities.
- Ability to work independently in a fast-paced environment.
- Current NDIS Worker Screening Check.
- Current Working With Children Check, where applicable.
- Current Driver’s Licence and reliable access to a vehicle.
- Right to work in Australia.
