- Experience within Support at Home, Home Care Packages, NDIS or disability services.

These aren’t traditional case management roles. You’ll work closely with
Care Partners, taking responsibility for much of the coordination, plan management and administration that sits behind the delivery of a client’s NDIS services.
It’s a great opportunity for someone who understands either Support at Home or the NDIS and is highly organised, administratively strong and comfortable managing lots of moving parts.
What you’ll be doing
- Supporting Care Partners with the administration and coordination of client plans
- Managing services against approved plans and budgets
- Coordinating services and providers
- Maintaining accurate client records and documentation
- Monitoring plan utilisation and identifying issues
- Following up actions and ensuring services are delivered as planned
- Liaising with internal teams, clients and external providers
- Taking ownership of the administrative detail that keeps client services running smoothly
We’re looking for:
- Experience within Support at Home, Home Care Packages, NDIS or disability services
- Strong administration and coordination skills
- Excellent attention to detail
- Confidence working across different systems
- Good understanding of plans, budgets and service coordination
- Strong communication skills
- Someone who is genuinely organised and enjoys getting things done
You don’t necessarily need to have held the title of Support Coordinator before. We’re particularly interested in people who understand the sector and have the organisation, systems and administrative capability to support a busy Care Partner team.
