RN For in Home Disability Complex Care & Clinical Team Leader Position.  

Country To Coast Care. Hunter and Newcastle.

 

We are seeking a qualified and passionate Registered Nurse for a dynamic role within our in- home disability support service. The Country to Coast Care Registered Nurse, will provide direct clinical support to our clients as well as provide leadership and industry standard training to our growing support team. 

 

The RN will be required to apply best practice, evidence-based procedures for:

Client Clinical Care: 

·      Supporting clients to maintain independence and the best possible quality of life.

·      Providing quality in home nursing care.

·      Providing clinical services such as, but not limited to, medication management,    

             wound care, bladder/catheter and bowel care and promoting continence. 

·      Conducting clinical assessments.

·      Responding to clinical care incidents. 

·      Developing, evaluating and reviewing individual care plans. 

·      Identifying individual changing care needs and conducting re-assessments.

·      Liaising with the client’s professional care team with the aim of achieving collaborative care outcomes.

 

Clinical Team Leadership:

·      Providing general clinical learning and development to the Country to Coast Care team in line with industry standards and company values.

·      Providing advice and direction to support workers in response to clinical incidents on shift.  

·      Providing clinical training for NDIS classified complex care tasks and ensuring staff are compliant and competent to provide this level of care. 

·      Ensuring clinical documentation policies and practices are maintained and compliant within the home environment. 

·      Monitoring medication assistance procedures in the home and ensuring practices are industry compliant. 

 

Experience: 

·      Degree in Nursing & current AHPRA registration as an RN.

·      At least 12 months post graduate experience is required. 

·      Demonstrated quality clinical skills in the areas of, but not limited to, medication and wound management, bladder/catheter and bowel care and continence promotion.

·      Demonstrated ability to work independently and autonomously and to provide timely and responsive clinical care and in-home clinical visits.   

·      Demonstrated work ethic that involves communicating collectively with a person’s professional care team.  

·      Of particular interest, would be applicants who have worked within the disability sector and who have knowledge of the disability industry standards and legislative practices.

·      Of particular interest would be applicants who have an understanding of, and experience in the management of clients living with Spinal Cord Injury. 

·      RN’s who have strong clinical experience, knowledge and a passion for providing quality care and learning but, may not be familiar with disability industry standards are also encouraged to apply.

These exciting new roles hours can be reasonably flexible for the right applicant but, must accommodate client clinical visit needs and staff availabilities for training. It is our aim that the Country to Coast Care RN will expand this position as they develop our organisational capacity to provide the more complex-care levels of support. As a result, the RN will be required to clinically support and respond to the growth of our complex-care client base.
