Description

At Chosen Family, we believe everyone deserves the opportunity to live with dignity, develop independence, and feel a genuine sense of belonging.

We are committed to creating an inclusive, community-centred environment where people are respected for who they are and supported to pursue meaningful lives. Through person-centred, trauma-informed, and recovery-oriented practice, we help individuals build skills, strengthen connections, and participate more confidently in their communities.

We are seeking compassionate, reliable, and experienced male-identifying Support Workers in Adamstown and the greater Newcastle area to express interest in an upcoming support opportunity involving community access, capacity building, and community reintegration.

If you are passionate about helping people develop independence, establish positive routines, and work towards their personal goals, this could be an opportunity to make a meaningful difference.

About the Opportunity

Chosen Family is preparing to support a 29-year-old participant with schizophrenia and cerebral palsy as he transitions back into the community following his anticipated release from custody.

The anticipated release date is 13 December 2026, subject to confirmation. We are inviting Expressions of Interest from suitable Support Workers who may be available to commence in December 2026 and provide consistent, respectful, and empowering support.

This opportunity goes beyond accompanying someone to appointments or assisting with daily activities. It involves helping a person establish routines, access community resources, build practical life skills, and work towards greater independence at their own pace.

You will support the participant to engage in activities of his choice while maintaining professional boundaries, following agreed support plans, and promoting safety, dignity, and self-determination.

Male-identifying Support Workers are particularly encouraged to express their interest, subject to the participant’s preferences and the final support arrangements.

What You’ll Be Supporting

Community Reintegration and Routine Building

  • Support the participant in establishing practical daily routines following his transition into community living.
  • Encourage participation in meaningful activities and community-based programs.
  • Provide consistent, respectful support while allowing the participant to make his own choices.
  • Help build confidence and familiarity with local services, facilities, and community resources.

Community Access and Appointment Support

  • Accompany the participant to appointments, errands, and activities aligned with his preferences and support plan.
  • Assist with attendance at Community Corrections appointments and allied health appointments, as required.
  • Support access to local amenities, recreational activities, and social opportunities.
  • Promote safe, planned, and positive community participation.

Capacity Building and Daily Living

  • Assist with grocery shopping, errands, and other practical daily living activities.
  • Encourage the development of decision-making, planning, budgeting, and independent living skills, as appropriate to the participant’s goals.
  • Support the participant to build confidence in managing everyday responsibilities.
  • Promote independence rather than unnecessary reliance on support.

Health, Wellbeing, and Positive Activities

  • Encourage participation in fitness, recreational activities, and healthy routines based on the participant’s interests.
  • Support access to appropriate community facilities and activities.
  • Promote social connection, emotional wellbeing, and meaningful engagement.
  • Respect the participant’s choices, abilities, and preferred pace of progress.

Professional and Consistent Support

  • Build a respectful, professional working relationship based on trust, clear communication, and appropriate boundaries.
  • Follow individualised support plans, risk management strategies, and organisational procedures.
  • Recognise and communicate emerging concerns to the appropriate team members.
  • Maintain accurate shift notes and relevant documentation.
  • Work collaboratively with the broader support team to promote continuity and consistency.

Who We’re Looking For

We are particularly interested in Support Workers who bring a calm, mature, respectful, and solutions-focused approach to their work.

You will ideally have:

  • Experience supporting people with psychosocial disabilities or complex support needs.
  • An understanding of schizophrenia, recovery-oriented practice, and mental health support.
  • Experience supporting people through significant life transitions, including community reintegration or justice-related support.
  • A trauma-informed approach that recognises the importance of dignity, choice, emotional safety, and trust.
  • The ability to maintain professional boundaries and follow support plans consistently.
  • Strong communication skills and the ability to remain calm and professional when situations become challenging.
  • A genuine commitment to promoting independence, capacity building, and meaningful community participation.
  • Flexibility to accommodate support hours that are yet to be finalised.
  • The ability to travel to Adamstown and the greater Newcastle area.

Experience supporting people with cerebral palsy, substance-use recovery, housing instability, or transitions from custody will be highly regarded.

An interest in fitness, recreation, and healthy lifestyle activities would also be advantageous.

What Makes You a Great Fit?
