We are establishing a new Isaac Mobile Support Program and are seeking expressions of interest from experienced professionals who are passionate about supporting young people to build safe, stable and independent futures.

This is an exciting opportunity to join a new mobile outreach service supporting young people across the Isaac Region, including Moranbah, Dysart, Middlemount, Clermont and surrounding communities.

About the Program

The Isaac Mobile Support Program delivers strengths-based, trauma-informed and culturally safe case management to young people who are homeless, at risk of homelessness, experiencing housing instability, or requiring practical support to develop independent living skills.

Working within a mobile outreach model, you will support young people to:

  • Secure and maintain safe, stable housing.
  • Build independent living and life skills.
  • Connect with local services and natural supports.
  • Work towards goals identified through an individualised case plan.

Services may be delivered from CatholicCare offices, partner agencies, schools, community locations or other safe environments, with all outreach undertaken in accordance with risk assessment and lone worker procedures.

What You’ll Be Doing

Key responsibilities include:

  • Deliver assertive outreach, engagement, intake, assessment and case management.
  • Develop, implement and regularly review individual case plans.
  • Support young people to obtain and sustain housing through advocacy and practical assistance.
  • Build strong partnerships with housing providers, schools, health services, community organisations and other stakeholders across the Isaac Region.
  • Deliver inclusive, culturally safe and person-centred support to young people from diverse backgrounds.
  • Travel throughout the Isaac Region to provide outreach services, with occasional after-hours work where required.

You will also demonstrate experience or knowledge of:

  • Trauma-informed practice

  • Culturally safe service delivery
  • Building collaborative relationships across community services

What You’ll Need

  • Diploma qualification in Community Services, Youth Work, Social Work, Human Services or a related discipline or Certificate IV qualification with substantial relevant equivalent experience.
  • Current Queensland Driver Licence.
  • Ability and willingness to travel throughout the Isaac Region.
  • Ability to obtain and maintain Working with Children Check (Blue Card), NDIS Worker Screening Clearance and satisfactory Australian Federal Police check.
