We are looking for an experienced and compassionate Registered Nurse to join our team as a Care Coordinator.

This is an exciting opportunity to be part of the establishment and growth of our Melbourne branch and help us deliver the same responsive, relationship focused care that At Home Together is known for.

We are looking for someone who understands that good care coordination is not just about paperwork and budgets. It is about knowing your clients, listening to families, identifying changes early and putting things in place to support the ageing population when they need it the most!

About the Role

As our Registered Nurse – Care Coordinator, you will manage and coordinate services for older people through the Support At Home program.

You will work closely with clients, families, support workers, allied health professionals, hospitals and other local service providers to ensure each client receives responsive, safe and appropriate support based on their individual needs.

This position combines Care Coordination, clinical oversight, client engagement and service planning.

Key Responsibilities

– Coordinate services for a portfolio of aged care clients.

– Conduct client assessments, review and regular face to face check ins.

– Develop and review individualised care and support plans.

– Identify clinical risks, deterioration and changes in client needs.

– Provide appropriate clinical oversight and guidance.

– Coordinate personal care, domestic assistance, social support and other in home services.

– Coordinate referrals for physiotherapy, occupational therapy, podiatry, nursing and other allied health services.

– Liaise with GP’s, hospitals, allied health professionals, families and external providers and stakeholders.

– Source appropriate equipment, assistive technology and home modification pathways where required.

– Support clients and families through hospital discharges and changes in care needs.

– Monitor client budgets and ensure services remain sustainable.

– Facilitate and support reassessment and review processes where increased care needs are identified.

– Maintain accurate records and documentation.

– Respond promptly to incidents, clinical concerns and safeguarding matters.

– Work collaboratively with rostering, administration and support team.

– Build strong, trusting relationships with clients and their families.

About You!

We are looking for someone who is proactive, organised and genuinely cares about people, first and foremost!

Essential Requirements

– Current registration as an RN with AHPRA.

– Experience in aged care, community nursing, home care or care coordination.

– Strong understanding of clinical risk and deterioration in older people.

– Excellent communication and relationship building skills.

– Strong assessment and documentation and care planning skills.

– Ability to manage multiple clients and completing priorities.

– Confidence communicating with clients, families and health professionals.

– Current National Police Check – or willing to obtain.

– Current drivers licence, registered and insured motor vehicle.

– Valid Australian Working rights.
