Clinical Pharmacist

One Ashford Hospital | FlexiBank – Hours as and when required.

We are seeking a dedicated Clinical Pharmacist to join our Pharmacy team on a FlexiBank arrangement at One Ashford Hospital, part of Phoenix Hospital Group. This role offers a unique opportunity to provide pharmaceutical care and support to patients, ensuring the safe and effective use of medications. As a Clinical Pharmacist, you will play a vital role in enhancing patient outcomes and collaborating with healthcare professionals in a dynamic medical environment.

Responsibilities:

  • Conduct comprehensive medication reviews and assessments for patients.
  • Provide expert advice on medication management to healthcare professionals.
  • Monitor patient responses to medications and adjust treatment plans as necessary.
  • Educate patients on the safe and effective use of their medications.
  • Collaborate with multidisciplinary teams to optimise patient care.
  • Maintain accurate and up-to-date patient medication records.
  • Participate in clinical audits and quality improvement initiatives.

Qualifications:

  • Bachelor’s degree in Pharmacy or equivalent.
  • Registered with the General Pharmaceutical Council (GPhC).
  • Minimum of 1 year of experience in a clinical pharmacy setting.
  • Strong knowledge of pharmacology and medication management.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team and independently.
  • Proficient in using pharmacy management systems and electronic health records.
