Description

Summary:

The Care Coordinator will be responsible for coordinating clients’ care services, advocating for their needs, and ensuring effective communication between healthcare professionals, client, and their families or caregivers.

Responsibilities:

  • Coordinate and manage client care services based on individual needs and resources
  • Conduct comprehensive assessments and develop individual care plans
  • Collaborate with healthcare professionals and other care coordinators
  • Advocate for clients’ access to necessary health services and resources
  • Schedule and monitor appointments, referrals, and care transitions
  • Communicate with clients, carers, and families to provide guidance, management, and care strategies
  • Maintain accurate client records and care plans
  • Participate in team meetings and quality improvement initiatives
  • Coordinate the overall care team
  • Other duties as assigned.

Qualifications:

  • NVQ 3 – 5 or working towards
  • Minimum of two years of experience in a senior healthcare or care coordination position
  • Knowledge of healthcare delivery systems and care coordination techniques
  • Excellent organizational, communication, and interpersonal skills
  • Ability to work independently or as part of a team
  • Strong analytical and problem-solving skills
  • Commitment to person-centered care
  • Proficiency in electronic health records and Microsoft Office Suite
