About the Role We are seeking compassionate and motivated Support Workers – Psychosocial to join our team and provide person-centred support to individuals living with mental health and psychosocial disabilities. In this role, you will help participants build independence, develop life skills, and improve their overall wellbeing while supporting them to achieve their goals.

Key Responsibilities

  • Provide day-to-day psychosocial support to participants in line with individual support plans
  • Assist participants with community participation, social engagement, and skill development
  • Support participants with daily living activities and routine management
  • Encourage independence, confidence, and self-advocacy
  • Monitor participant wellbeing and report any changes or concerns
  • Maintain accurate documentation and progress notes

  • Work collaboratively with families, healthcare professionals, and support coordinators

  • Desirable:

    • Experience in wound care
    • Experience in Mental Health
    • Proven experience in managing severe dysphagia, including administering modified meals and liquids.
    • Expertise in providing high-level care, including medication administration and feeding assistance.
    • Proficiency in hoist transfers and ensuring safe manual handling practices.
    • Demonstrated skills in aspiration management and implementing preventative measures.
    • Strong understanding of individual care plans and delivering person-centered support.

    Qualifications:

    • Certificate III in Disability or equivalent experience or working towards it
    • Experience working in Disability Care within the community setting.
    • Leadership experience in aged care or disability care, with the ability to influence others to deliver outstanding care and customer service.
    • High level of clinical knowledge.
    • Sound understanding of Aged Care and Disability Quality Standards and quality management systems.
    • Full Australian working rights and ability to pass a national police clearance.
