Disability & Mental Health Support Worker

Hourly Rate: $38 – $45 per hour (based on qualifications and experience)

Due to continued organisational growth, we are seeking dedicated and experienced Disability and Mental Health Support Workers to join our expanding team. This role offers approximately 25+ hours per week, with the potential to transition to full-time employment.
The Role
As a Support Worker with Mindset Intervention, you will provide individualised, community-based and in-home support aligned with each participant’s NDIS goals. Duties include (but are not limited to):
Assisting with personal care and hygiene
Completing session reports and maintaining accurate documentation in line with NDIS requirements.
Domestic assistance (cleaning, laundry, etc.)
Everyday tasks such as budgeting, shopping, and meal preparation
Supporting participants to attend appointments and community activities
Implementing behaviour support plans and working toward participant goals
Transporting participants safely to and from activities
Medication prompting
Certificate III in Disability, Individual Support, or a related field (or willingness to obtain).
Collaborating with team members to ensure continuity of care
Demonstrated experience in implementing behaviour support plans and providing person-centred support in alignment with NDIS goals.
Understanding of relevant legislation and standards within the disability and mental health sectors.
Previous experience working with individuals with complex behaviours.
Adhering to organisational policies and procedures
Participating in sleepover shifts as required
Requirements
NDIS Worker Screening Check (or willingness to obtain)
First Aid & CPR Certificate
NDIS Worker Orientation Module (or willingness to obtain)
COVID-19 Infection Control Training (or willingness to obtain)
COVID-19 Aged Care PPE Module 5 (or willingness to obtain)
Paid Blue Card (if required)
Current Australian Driver Licence
Reliable, registered, and comprehensively insured vehicle
Certificate III in Disability or Mental Health (or willingness to obtain)
Smartphone with data access
Flexible availability (including weekends and evenings)
Excellent communication skills and a calm, professional manner
