Description

Job Title: Coordinator – Community Care (Disability Services)

Job Summary:

We are seeking a dedicated Coordinator to join our Community Care team in Karratha, Western Australia.

The successful candidate will be responsible for coordinating community care services to ensure the wellbeing of our participants with disabilities. This role requires strong organizational skills, attention to detail, and a passion for helping others.

Skills:

– Excellent communication and interpersonal skills

– Strong organizational abilities

– Ability to work effectively in a team environment

– Knowledge of (NDIS), community care services and resources

– Experience with care planning and rostering

Responsibilities:

– Coordinate community care services for clients, including scheduling appointments and arranging transportation

– Conduct client assessments to determine their needs and execute the NDIS care plans

– Monitor the delivery of care services to ensure quality standards are met

– Collaborate with other healthcare professionals to provide comprehensive support for clients

– Coordinate with relevant stakeholders for the welbeing and safety of the participants

– Maintain accurate records of client information and service delivery

Qualifications:

– Diploma in Community services, social work, nursing, or related field

– Previous experience in community care or similar role preferred

Experience:

1+ years of experience in a similar role is preferred.
