This role requires someone who is patient, calm, safety-focused, and experienced in complex care environments.Medical Conditions & Support Needs:Huntington’s DiseaseHigh falls riskSchizophreniaFaecal and urinary incontinenceMeal preparation and supervision in line with a modified diet and mealtime management planBehavioural Considerations:Occasional verbal aggressionRefusal of personal care or medication at timesSmoker requiring supervision due to risk of self-injuryKey Responsibilities:Personal Care & Medication Support: Assist with hygiene, continence care, and medication adherence using positive reinforcementMeal Preparation & Monitoring: Prepare meals according to dietary requirements and ensure safe mealtime practicesCompassionate 1:1 Support: Build trust and provide respectful, person-centred carePromoting Independence: Encourage autonomy and engagement in meaningful activitiesHealth Monitoring & Documentation: Observe, record, and report changes in physical or emotional wellbeingShift Availability: MUST BE ABLE TO DO ALEEPOVER SHIFT (2 PM – 6 AM ) Flexibility across shifts is highly regarded.About You:Compassionate, patient, and respectfulCalm and resilient when managing challenging behavioursExperienced in complex care and behavioural supportSafety-focused, especially around falls risk and smoking supervisionStrong communicator with a consistent and professional approachEssential Requirements:Certificate III or IV in Disability, Aged Care, or equivalentCurrent NDIS Worker Screening CheckCurrent Employee Working with Children CheckNational Police Check (or willingness to obtain)Valid Australian driver’s licenceReliable vehicle with comprehensive insuranceCurrent First Aid & CPR certificationProven experience supporting individuals with complex behaviours and disabilitiesHighly Desirable:Experience supporting individuals with Huntington’s Disease, high falls risk, and incontinence careReady to Make a Difference?If you’re passionate about empowering individuals and providing care with dignity and compassion, we’d love to hear from you.👉 Apply now and become part of the Connecting Families team — together, we create brighter futures, one step at a time.
