Role Responsibilities:

  • Coordinate both formal and informal supports
  • Manage clients with complex needs and handle crisis situations
  • Collaborate with a Multidisciplinary Panel
  • Engage with Mental Health, Justice System, Housing, and Health sectors
  • Undertake clerical administration and report writing
  • Network and communicate with key stakeholders
  • Work independently, either from home or on-site.
  • Work in a sensitive, complex and demanding environment with a high degree of autonomy and self-management.

Qualifications & Experience:

  • Minimum of Certificate IV in Community Services or higher (or a related discipline and/or relevant experience and knowledge.)
  • Two or more years experience working with participants with any of the following: Mental health (moderate to complex), high medical needs, behavioural support needs
  • Experience in Forensic Disability or Forensic Mental Health is a plus
  • Demonstrated knowledge of the NDIS and experience working with its participants
  • Valid Driver’s License with a suitable, insured vehicle
  • A current Working with Vulnerable People registration
  • Recent police check (or willingness to obtain one).
