Care Coordinator

Summary:

The Care Coordinator will coordinate care services and ensure effective communication between healthcare professionals, patients, their families and caregivers.

Responsibilities:

• Coordinate and manage care services based on individual needs and resources
• Conduct comprehensive assessments and develop individual care plans
• Collaborate with healthcare professionals and other care coordinators
• Communicate with patients and families to provide guidance
• Maintain accurate patient records and care plans
• Participate in team meetings and quality improvement initiatives
• Coordinate with the local authority and safeguarding board
• Support with recruitment

Qualifications:

• Minimum of two years of experience in healthcare or care coordination
• Knowledge of healthcare delivery systems and care coordination techniques
• Excellent organisational, communication, and interpersonal skills
• Ability to work independently or as part of a team in a fast-paced environment
• Strong analytical and problem-solving skills
• Commitment to patient-centred care and empathy for service user needs
• Proficiency in care terminology, electronic health records, and Microsoft Office Suite
