THE ROLE

A service coordinator’s job description typically involves managing and coordinating the delivery of services, ensuring customer satisfaction, and streamlining communication and logistics between clients and service providers. They are responsible for supporting Focus Care operations ensuring consistent and high-quality service provision.

The Service Coordinator will be directly responsible for managing incoming enquiries from existing clients, daily management of client service plans, responding to ad hoc requests, advocating for our clients and implementing changes with the direction of the Service Operations Manager and in collaboration with Care Coordinators.

Our goal is to ensure we can redefine traditional healthcare and ensure our clients and participants can live independently in their home respecting their choice, control and dignity.

The Service Coordinator will adopt a best practice customer service approach – to assist in accessing available and relevant resources to meet their goals. KEY ACCOUNTABILITIES

You will be responsible for:

  • Planning and scheduling: Coordinating service delivery, including scheduling appointments, managing resources, and ensuring timely service provision.
  • Client Liaison: Acting as a point of contact for clients, addressing their inquiries, and resolving issues. Building and maintaining strong relationships with clients to ensure satisfaction.
  • Communication: Facilitating clear and effective communication between clients, service providers, and internal teams.
  • Service Delivery: Ensuring that services are provided according to the client’s needs and the organization’s standards.
  • Problem Solving: Identifying and resolving client issues promptly and efficiently, gathering client feedback to identify areas for improvement.
  • Record Keeping: Maintaining accurate records of client interactions, service delivery, and other relevant information. Managing client data and ensuring data privacy and security.
  • Advocacy & Collaboration: Acting as an advocate for clients and their needs. Working collaboratively with other departments and stakeholders to ensure seamless service delivery.
