About the Role
The Support Coordination team is part of our Fortify Health Foundation, a non-for-profit entity within Fortify Health Group providing support to resolve complexity and improve overall health and wellbeing. Come and join our NDIS Support Coordinator team and embark on a rewarding journey supporting individuals and families in implementing their NDIS plans. The Support Coordination team is responsible for connecting people with a National Disability Insurance Scheme (NDIS) Plan to access a range of supports which align with the support items in their NDIS plan and that suit their individual needs, across one or more providers. As a Support Coordinator, you will provide varying levels of coordination and transition support for ages 0 to 65 years old. As well as considering individual’s circumstances and level of complexity. Participants will be matched with community based and/or mainstream programs to fulfil their individual needs whilst receiving continuous support and assistance. We would consider applications who wish to engage under a sub-contract arrangement where preferred.

To be successful you must posses
- A diploma or tertiary qualification in degree in Human Services, Social Work or related disciplines
- Demonstrated strong time management skills including prioritisation of competing tasks
- Demonstrated experience in the provision of support and coordination for people with a lived experience of a mental health and other disabilities.
- Experience working with culturally and Linguistically Diverse persons.
- In-depth understanding of the NDIS and experience working within NDIS sector for a minimum of two years.
- High level of interpersonal and communication skills to build rapport and interact effectively with clients, staff and other agencies
- Experience in preparing detailed reports as per NDIS requirements.
- Computer Literacy, word processing and database competencies.
- Current Queensland driver’s license.
- A Paid Blue card or the ability to obtain one is essential
- NDIS worker screening check or the ability to obtain one is essential
- Rights to work in Australia
