Position Overview

Crosstown Family Health Team (FHT), in partnership with the Weston Family Health Organization, delivers comprehensive, team-based primary care to a diverse, rostered population. Services focus on chronic disease management, health promotion, illness prevention, and the social determinants of health. Our inter-professional team works in a collaborative, patient-centered model.

We are currently seeking a Registered Nurse (RN) with additional responsibilities in System Navigation. This is a full-time position combining direct clinical care with a focused effort to support patients facing complex social and health challenges. The ideal candidate is adaptable, collaborative, and committed to improving health outcomes in the community.

Key Responsibilities Clinical Nursing Duties (80%)

  • Conduct initial patient assessments and histories
  • Administer immunizations and manage vaccine inventory
  • Provide women’s health services including Pap smear screenings
  • Perform clinical tasks such as:
  • Wound care and dressings
  • Suture/staple removal
  • TB skin tests
  • Allergy & B12 injections
  • Liquid nitrogen application
  • Ear syringing
  • Preventative Screenings (PAPs)
  • Support physicians during clinical procedures
  • Deliver health education and self-management support for chronic conditions (e.g., diabetes, COPD, mental health, senior care)
  • Facilitate group education sessions and smoking cessation counselling both in-person and virtually
  • Collaborate with the inter-professional team to coordinate comprehensive patient care

System Navigation (20%)

  • Accept and triage referrals from physicians and team members
  • Assess patient needs including housing, income, food security, mental health, and access to services
  • Develop individualized care/service plans with clear goals and timelines
  • Support patients in navigating the health and social service systems
  • Connect patients with appropriate community resources using tools like Toronto Central Health Line and 211Toronto
  • Advocate on behalf of patients and support their self-advocacy
  • Participate in case conferencing and interdisciplinary planning
  • Educate staff and patients about community services and supports

Additional Responsibilities

  • Collect and maintain data for reporting and evaluation
  • Contribute to monthly and quarterly reports for Ontario Health
  • Participate in team meetings, quality improvement initiatives, joint health and safety programs, and program development
  • Follow all internal policies, procedures, and professional standards

Qualifications & Requirements

  • Bachelor’s degree in Nursing from a recognized university
  • Registered Nurse in good standing with the College of Nurses of Ontario (CNO)
  • Current Basic Cardiac Life Support (BCLS) certification
  • Experience in primary care, community health, or inter-professional teams (Family Health Team, CHC, CCAC/OHH, etc.)
  • Registration and Professional Insurance Liability Protection with RNAO
  • Experience in chronic disease management and smoking cessation preferred
  • Additional certifications an asset (e.g., Certified Diabetes Educator, Certified Respiratory Educator, Craving Change)
  • Knowledge of community health resources and social determinants of health
  • Excellent communication, interpersonal, organizational, and computer skills
  • Experience facilitating health education groups
  • Demonstrated ability to work independently and collaboratively in a fast-paced, changing environment
  • Flexible availability including one evening or weekend shift
  • Culturally competent with experience serving diverse and vulnerable populations
