Our operating model

thePHO is being built as a lean, practice-first PHO with a clear community health purpose. We will combine national capability, regional partnership, and smart shared infrastructure to strengthen general practice, improve access and equity, and deliver better outcomes for the communities we serve.

Our model is designed to support general practice as the frontline foundation of primary care, while fulfilling the broader role of a PHO in improving population health, enabling prevention and early intervention, and helping coordinate care across community, primary, and wider health services.

We will focus on the functions that add genuine value at scale. Funding flow, contracting, digital enablement, data and reporting, service support, and transition management. In doing so, we will reduce unnecessary bureaucracy, preserve what works locally, and direct more value to frontline care.

It is a modern PHO designed to improve community outcomes through strong general practice, practical partnerships, and efficient shared capability.

Practice first — Manaakitanga

We start with general practice. Decisions are shaped by what works on the ground and what improves care for patients.

Lean and purposeful — Haepapa, Whaihua

We keep the centre lean and focused. Funding and effort go to the frontline. We remove duplication so more reaches practices and communities.

Clinically grounded — Pono

We base decisions on day to day clinical practice. We work alongside clinicians and back them with practical, consistent support, leading to better care and outcomes.

Equity centred — Tika, Te Tiriti o Waitangi

We apply Te Tiriti in how we fund, support, and measure care. We improve access and outcomes for Māori and all communities.

Connected and enabling — Whanaungatanga, Auahatanga

We align across Health NZ, IMPBs, PHOs, and partners. We use systems and data to reduce admin and make care easier to deliver, resulting in more coordinated care.
