Position Title: Registered Nurse (RN) – Home Health

Reporting To: Clinical Nurse Lead with daily collaboration with the wider Wattle and River care team

Location: Goondiwindi, QLD and surrounding areas

Employment Type: As per contract (casual/part-time/full-time)

Role Purpose

The Registered Nurse (RN) plays a central role in delivering Wattle and River Home Health’s mission of high-quality, community-based aged care and chronic disease management. Working autonomously and collaboratively, the RN provides compassionate, evidence-based clinical care to support clients to remain well, safe, and independent in their own homes. This role includes clinical leadership, assessment, care planning, mentoring support staff, and acting as an advocate for clients' wellbeing, dignity, and choices.

Key Responsibility Areas & Performance Measures

Clinical Care and Health Assessment

• Conduct nursing assessments and reassessments aligned with care goals and clinical need.

• Implement and update person-centred care plans based on current clinical findings.

• Monitor clients’ health, identify deterioration, and escalate or refer as appropriate.

• Administer medications and treatments safely in accordance with best practice.

Nursing Leadership and Supervision

• Provide clinical guidance and informal training to Personal Care Assistants and Enrolled Nurses.

• Participate in onboarding, competency checks, and reflective practice sessions.

• Model respectful, high-quality, person-centred care at all times.

Care Coordination and Communication

• Liaise with allied health professionals, GPs, families, and the broader care team.

• Contribute to multidisciplinary case conferences and discharge planning.

• Keep clients informed and engaged in decisions about their care.

Safety, Quality, and Continuous Improvement

• Comply with infection control, medication safety, and manual handling procedures.

• Participate in audits, incident investigations, and quality initiatives.

• Report clinical risks and near misses promptly.

Holistic and Emotional Support

• Build trusting relationships with clients and families.

• Acknowledge grief, loss, and transitions with sensitivity.

• Promote mental, emotional, and social wellbeing through meaningful engagement.

Working Conditions and Availability

This is a shift-based role delivering care to clients in their homes in Goondiwindi, QLD, and surrounding areas.

As part of our rural and regional model of care, the role requires flexibility, adaptability, and a commitment to community-centred health.

RNs must be available to work morning, evening, and overnight shifts, including weekends and public holidays as rostered.

Shifts may be 8, 10, or 12 hours in duration, depending on service and client needs.

Some travel between client homes may be required. A current driver's licence and reliable vehicle are essential.

This role may suit nurses who are already living in or relocating to rural or regional Queensland, or those seeking meaningful work in a community setting.

Essential Criteria

• Registered Nurse with AHPRA registration.

• Proven experience in aged care, community health, or chronic disease management.

• Strong clinical assessment and decision-making skills.

• Excellent interpersonal and written communication skills.

• Digital literacy and confidence using care software (or willingness to learn).

• Current Police Check or NDIS Worker Screening Clearance.

• Current Driver’s Licence and reliable transport.

Desirable Criteria

• Experience with Home Care Packages (HCP), CHSP, or NDIS.

• Postgraduate studies in community, palliative, or gerontological nursing.

• Familiarity with culturally safe practice, trauma-informed care, or inclusive models of care.
