At Power of Grace Care Services (POGCS), we believe that care is more than a service. Joining POGCS means becoming part of a family — one that laughs together, learns together, and supports each other through challenges. Our culture is built on cooperation, multicultural respect, and the belief that everyone has a role in building a more inclusive community.
What each role focuses on and how it helps empower our participants.
Build capacity for everyday living: functional assessments (FCA), goal-based interventions, AT/equipment prescription and training, home mods recommendations, sensory/participation support, and collaborative planning with families and teams.
Reduce behaviours of concern and improve quality of life: functional behaviour assessment, Positive Behaviour Support Plans, capacity-building strategies, carer training, data review, and restrictive practice minimisation in line with the NDIS Commission.
Improve mobility and physical wellbeing: gait/balance assessment, strength and conditioning, pain and posture management, falls prevention, and mobility/positioning equipment recommendations.
Deliver therapist‑designed programs under delegation and supervision: session carryover, data collection, home/school/community practice, and feedback loops to the supervising clinician.
Help participants understand and implement their plans: connect with providers, coordinate services, build capacity and problem‑solve barriers, monitor budgets, and provide required reports/updates.
Community‑based nursing for disability‑related health needs: medication support, wound/skin care, continence management, health education, and collaborative planning with the broader team (role requires appropriate registration and clearances).
Practical, day‑to‑day support: personal care, domestic assistance, community access, transport, skill‑building, and consistent implementation of therapy and behaviour strategies.
The practical support, flexibility, and growth pathways that help you do your best work and feel good doing it. These are the things clinicians and support staff tell us matter most day‑to‑day.
