Holyoake’s Wheatbelt Community Alcohol and Drug Service are seeking a passionate and empathetic health professional to support individuals experiencing suicidal ideation or suicide behaviour throughout the Wheatbelt in the position of After Care Coordinator in Suicide Prevention (ACC).

Using a case management and strength base model, the After Care Coordinator will be responsible for:

Promoting this free program aligned with the Western Australian Suicide Prevention Framework 2021-2025 throughout the Wheatbelt to clients, significant others, community mental health services, GPs, and hospital emergency staff.
Supporting individuals and significant others to recover from difficult life events that have contributed to suicidal thoughts and suicide attempts.
Assisting individuals to engage in support services to decrease suicide risk factors such as life stressors, trauma and mental health challenges by encouraging strong protective factors and coping skill development.
Developing meaningful partnerships that strengthen collective impact of suicide prevention in the Wheatbelt.

This Western Australian Primary Health Alliance funded position provides a place-based suicide prevention service in the Wheatbelt to arrange after care for people who have presented at hospital or GP clinic with suicide ideation or an attempt. The After Care Coordinator will facilitate a safe return to home and community by mobilising available community services and supports as identified in the hospital discharge plan and any other services relevant to the client’s needs.

This position will be based in Northam and involves outreach throughout the Wheatbelt region. Consideration will be given to suitable applicants living in or within close proximity to the Wheatbelt. The After Care Coordinator will work closely with the Wheatbelt Suicide Prevention Coordinator, Holyoake Wheatbelt Community Alcohol and Drug Service staff and key stakeholders within the Wheatbelt.

Skills required:

A tertiary qualification in social work, psychology, mental health (nursing), counselling or equivalent experience.
Demonstrated experience in working to support individuals with mental health concerns and suicidal ideation.
Demonstrated experience in conducting suicide risk assessments, de-escalation, and the development and implementation of client safety plans.
Demonstrated experience in complex case management.
Sound knowledge of alcohol and drug issues, mental health conditions and their comorbidity.
Minimum 4 years professional experience.
