Key Responsibilities
- Deliver a range of psychosocial assessment and intervention services
- Assessing, planning, implementing, monitoring and evaluating actions required to meet the participants’ health and human services needs
- Provide clear, timely and professional reporting to stakeholders
- Build and maintain strong relationships with clients, employers and referral partners
The Role
Undertake psychosocial assessment and intervention services to clientele across varying schemes including Life Insurance, NSW Workers Compensation, iCare and CTP
Core responsibilities of the role will include assessing, planning, implementing, monitoring and evaluating actions required to meet the participants’ health, wellbeing and social needs
What You’ll Need
- Full membership with AASW (Australian Association of Social Workers)
- Minimum 3 years experience in a clinical, rehabilitation or occupational rehabilitation setting
- A sound knowledge of compensable schemes including Life Insurance, NSW Workers Compensation, iCare and CTP
- Experience working with clients who have psychological injuries and/or complex needs and clients who may be verbally aggressive/hostile or may refuse to follow instruction
- Demonstrated ability and experience in case management, discharge planning, acute mental health and care coordination
- Comprehensive understanding of, and expertise in, connecting clients with government agencies and welfare supports, including but not limited to Centrelink, state-based public housing agencies (such as DCJ Housing NSW), NDIS, Legal Aid NSW, and NSW Trustee and Guardian
- Outstanding verbal and written communication skills
- Strong problem-solving abilities
- A current driver’s licence, access to your own vehicle and willingness to travel to provide face-to-face servicing of clients
- Confidence with Microsoft Office and electronic client record management
