About the organisation

He Waka Tapu is a Kaupapa Maori organisation who have been supporting individuals and the community for 30 years. He Waka Tapu works to increase the health and wellbeing of Whānau.

About the role

Te Tumu Waiora – te reo for ‘to head towards wellness’ – is a new model of primary mental health and addictions care and support which aims to provide all New Zealanders experiencing mental distress or addictions challenges with access to convenient, high quality, integrated and person-centred care and support. The model is being rolled out nationally over the next four years.

The model puts mental health and wellbeing at the heart of general practice with focused roles, Health Improvement Practitioners and Health Coaches, working as part of the general practice team. They also have access to a Support Worker who links the team and their patients to services throughout the community.

The Health Coach role aims to connect directly with patients to assist them to make any necessary changes to improve their health and wellbeing and work towards an achievable healthcare plan. The role involves being an integral part of the general practice team, working directly with individuals and their families to help improve their mental health and wellbeing.

You may be required to work in several settings across Canterbury in general practice, the community and within our Kaupapa organisation.

Skills and experience

  • Experience with mental and/or physical health condition or have been /are a caregiver for someone with health condition (s) and/or have interest in managing health and well-being
  • Ability to learn and apply self-management skills
  • Excellent interpersonal skills
  • Excellent English written and communication skills
  • Experience working with Maori, Pasifika and youth
  • Understand the importance of following evidence-based models, policies, practices and procedures
